Join the Foundation
LDF membership is open to clinicians, students, institutions and the public who share our work in diabetes awareness, education and welfare. Applications are reviewed by the Foundation. Approved members receive a unique Membership ID, a downloadable certificate, and a dashboard to renew each year.

From application to certificate
Select a category, add your photograph and submit the form for that path.
Your application sits in the LDF portal as Pending Approval until the team decides.
Approved members get an ID, issue and expiry dates, and a PDF certificate with a QR code.
Membership lasts one year. Renew from your dashboard; renewal also needs admin approval.
